CCH 2012 Poster Presentation Focuses on Quality Improvement The recently held Compounders on Capitol Hill 2012 featured IACP’s fifth annual IACP Science and Practice Poster Presentation Showcase. The poster presentation session offers the opportunity for IACP members, pharmacy professors and pharmacy students/residents to share information about topics including patient case reports and research in compounding.The compounding profession is built upon solving unique patient cases and IACP’s Science and Practice Poster Presentation Showcase is a great way to share cutting-edge research with your peers. The poster presentation showcase is open to IACP members, pharmacy professors and pharmacy students/residents. This year, IACP Member Dana Reed-Kane, PharmD, FIACP, of Reed’s Compounding Pharmacy in Tucson, Ariz., sponsored a poster, “E-Prescribing Errors Identified in a Compounding Pharmacy.” During the spring semester of 2012, four students from the University of Arizona College of Pharmacy conducted a project for their Quality Improvement class at Reed’s Compounding Pharmacy. After being frustrated with the amount of errors being observed with electronic prescriptions that were sent to the pharmacy via fax, the students’ preceptor, Dana Reed- Kane, suggested the idea of researching what kind of errors were made, as well as the error rate. Over the course of four weeks, the students reviewed a total of 110 electronic prescriptions and found an error rate of 63%.The most common error noted involved items being placed in the wrong entry field, for example, the name of the medication was placed in the, “directions of use,” area. The students displayed their results to students, faculty and preceptors at a poster presentation held at the College of Pharmacy. Reed-Kane presented the poster at IACP’s Compounders on Capitol Hill in Washington, D.C., in June. The results also are being presented to SureScripts, one the major software vendors for electronic prescriptions in hopes that they may be able to help improve the process.The project has now been submitted to the peer-reviewed International Journal of Pharmaceutical Compounding for possible publication.
