The Outcomes Personal Pharmacist Program is your passport to the knowledge and expertise of a personal pharmacist. 58% of all adults have a personal pharmacist to help them with their medication needs. Outcomes Partners+Plus Program™
Partners+Plus is a new program that combines the data mining features of your PBM with the Outcomes professional services menu to create a powerful health care management tool. Through Partners+Plus, your PBM identifies members of a client's health plan consuming 7 or more prescription medications. A certificate is mailed to the home of each of these members, offering a Comprehensive Medication Review by a specially trained Outcomes Personal Pharmacist™. During the Comprehensive Medication Review, the pharmacist will assess the member's drug profile for potential conflicts, therapeutic duplications, and formulary/generic savings opportunities. They will then consult with the member and/or their physicians, as appropriate.
