Product Overview

Dexcom CGM sends real-time glucose readings to a smart device or handheld receiver via a small sensor worn on the body. Dexcom CGM may help patients manage their diabetes following hospital discharge. The Dexcom hospital discharge program provides a trial of Dexcom G7 15 Day for adult patients or Dexcom G7 for pediatric patients discharged from the hospital who have been identified as appropriate candidates.

Dexcom’s hospital discharge program is designed to decrease readmissions. A prospective, nonrandomized study showed that initiating Dexcom CGM* at hospital discharge is associated with significant glycemic improvements in patients with type 2 diabetes on basal insulin. A1C decreased from 11.7% to 8.2% (P <0.0001). Time below 70 mg/dL was <1% at all times.3 Real-world evidence also demonstrates glycemic and emotional benefits of Dexcom CGM initiation at discharge. A1C decreased 3.2% after 12 weeks of Dexcom CGM use. Change in median Diabetes Distress Scale score improved significantly from 2.65 to 1.76 (P <0.001) after 12 weeks of Dexcom CGM use.4 With Dexcom CGM use, patients reduced their diabetes-related inpatient admission costs by 35%†,5 and reduced their diabetes-related inpatient visits by 50%.‡,6

Dexcom Clarity helps care teams stay connected with patients following hospital discharge and can keep providers updated between visits.
