Patient-Centered Pharmacy Home™ is a Value-Based Care Program Developed to Empower Pharmacists
Our proprietary Patient-Centered Pharmacy Home™ (PCPH) Accreditation program is a highly respected value-based care program designed specifically for pharmacists who are leaders in their communities. This program may help a pharmacy develop stronger relationships with referral sources and expand its fee-based services.
PCPH is based on our CMS-recognized Patient-Centered Medical Home Accreditation. The program focuses on advanced pharmacy practices and empowers pharmacy personnel to work at the top of their licenses and to play an active role on a patient’s care team.
Integrated Community Pharmacy/DMEPOS Accreditation Yields Exceptional Value
Most pharmacies, chain or independent, are accredited to bill Medicare for DMEPOS services and supplies. As such, we combined the DMEPOS requirements into our Community Pharmacy program — at no additional charge to you.
