Product Overview
Our new Clinical Disease Management program recognizes a pharmacy’s expanding role from a transaction-based to a clinical-based services practice.
The Compliance Team recently developed and launched a new program that focuses on clinical disease management (CDM). Pharmacies that achieve the CDM recognition are acknowledged for their roles in helping patients with diabetes, hypertension, and cardiovascular diseases improve their overall health outcomes and quality of life. The program encourages pharmacies to transform from a transaction-based practice to a services and value-based care practice that promotes continuous, proactive, and patient-centered care in the community.

Clinical Disease Management is available either as a sub-specialty of our Community Pharmacy program, the foundation of our pharmacy suite of services, or as a stand-alone certification program.
